ORIGINATION SECTION
Unshaded areas to be completed by the responsibility
center/department.
1. Account number (NN-NNNNN-NNNN-NNNNN-NNNNNN-NNNNN-NNNNN).
2. Name of the department, section, unit, or project associated with
the position.
3. Job Classification Title associated with the job classification
code (description from SPI 80).
4. Employment Status (check one):
RF Regular Full-Time -- A pre-determined schedule of hours and
days in a position established as essential for the long-
term operation of the department with a percent of effort of
100% of the established 37 1/2 hour work week, or where
applicable, the 40 hour work week.
RP Regular Part-Time -- A regular position as described in RF
above and with percent of effort at a pro rate of the
established work week of 37 1/2 or 40 hours, whichever is
applicable.
TF Temporary Full-Time -- A position limited and/or sporadic in
duration for a specific project, for the temporary
replacement of an absent regular employee, or for short term
needs during peak periods of work load. The percent of
effort is 100% of the established work week. Usually does
not exceed six months. Requires further review if beyond
six months.
TP Temporary Part-Time -- A temporary position as described in
TF above. The percent of effort is less than 100% of the
established work week and is proportionate to the
anticipated actual hours during the period of employment.

EX Exempt from overtime provisions of the Fair Labor Standards
Act.
N Non-exempt from overtime provisions of the Fair Labor
Standards Act.

11. Four-digit primary job classification code (see SPI 80 or Data
Element Dictionary (DED) Table 18324).
12. Percent of salary supported by University budgeted funds.
13. Percent of salary supported by other than University funds.
14. Name of person being replaced, if applicable.
15. Name, campus address, and campus telephone number of the person
within the department to be contacted regarding the request.
16. Name, campus address, and campus telephone number of the
immediate supervisor for the position.
