          Data Download

    2.    Check  one  to  indicate downloading to a  Diskette  or
          Extract

    3.    Name of Requester (Last, First, Middle Initial)

    4.    Requester's Department

    5.    The character length of the combined output lines

    6.    Number  of  unique "key" or entity records selected  on
          the Request for Data Download

    7.    The unique fields within the data record

    8.    For Extract Only: Dataset Name

    9.    For  Diskettes Only: 8 character name assigned  by  the
          download program.  See reverse side of FORM 0078

   10.    For  LUV Only: Check to indicate downloading by Logical
          User View.  Include name and description of view.
