    1.    Social Security Number of Student

    2.    Name of Student (Last, First, MI)

    3.    Check one to indicate Undergraduate or Graduate
          (Includes First Professional) Level

    4.    Year the Term Begins


Academic Program Codes

*    Academic Program codes must be active for the term of
     Registration.  Refer to ISIS       System Table SVAL for
     verification.

    5.    Description of Primary Academic Program (Refer to ISIS
          System Table SPRO if necessary.)

    6.    Code of Primary Academic Program from ISIS System Table
          SVAL

    7.    Description of Secondary Academic Program (Optional)

    8.    Code of Secondary Academic Program, if applicable, from
          ISIS System Table SVAL


Course Section

Enter the following information from the Schedule of Classes for
the appropriate term:

    9.    CRN (5 digit Course Reference Number) replaces what
          used to be know as the "Alpha Code".  This number
          uniquely identifies a section of a course.

   10.    Subject Area Abbreviation

   11.    Course Number (4 digits)

   12.    Number of Credits for which the course is being taken.
          (Credit hours may be listed in one-half credit
          increments, eg., 00.5, 01.5 or 03.5.  Courses may also
          be listed for variable credits, eg., 01.0 TO 06.0, 03.0
          OR 06.0.)

   13.    Building and Room Number where the course will be held

   14.    Day(s) the course will meet

   15.    Course Beginning and End Times

Alternate Course Section

*    Alternate course selections are required.  As popular
     courses reach course capacity, or as   the registration
     period progresses, entering approved alternate courses will
     expedite the   registration process.

   16.    Enter the required information from the Schedule of
          Classes as described above.

   18.    Advisor's Signature and Date required.
