    1.    Social Security Number of Student

    2.    Name of Student (Last, First, MI)

    3.    Check one to indicate Undergraduate or Graduate
          (Includes First Professional) Level

    4.    Year the Term Begins

    5.    Check one to designate Closed Class or Restricted
          Class.


Enter the following information from the Schedule of Classes for
the appropriate class:

    6.    CRN (5 digit Course Reference Number) replaces what
          used to be know as the "Alpha Code".  This number
          uniquely identifies a section of a course.

    7.    Subject Area Abbreviation (Up to 6 characters)

    8.    Course Number (4 digits)

    9.    Course Title

   10.    Instructor's Signature (if required) and Date.

   11.    Department Chariman's Signature (required) and Date.
