SUBJECT: Establishing University Forms

I. SCOPE
This policy governs the establishment and control of all University-wide forms required to
implement University procedures and includes:
- Initiation of new forms
- Revision of existing forms
This policy does not include those departmental (PITT Number Only) forms that are
unrelated to University Procedures.
II. POLICY
The Chancellor, as the chief academic and administrative officer, is responsible for the
approval of University forms. This responsibility is delegated to the Data Administrator to
ensure that all forms are consistent with University data requirements.
FORM Numbering
All forms associated with a University Procedure are identified by a FORM number,
assigned by the Data Administrator.
Upon association with a University procedure, existing STOCK numbered forms are
converted to the FORM numbering scheme.
When associated with a University procedure, Departmental Forms are assigned both a
FORM number and a PITT Number.
Inventory
FORM Numbered forms and Stock forms are stored in, and requisitioned from the
University Stockroom. (See Procedure 10-06-01, Stockroom Requisitions.)
Forms with both FORM and PITT numbers are stored in the individual departments.
