PRISM User Information Forms

For New PRISM Users and Changes to Existing PRISM Accounts

Please fill out the appropriate forms located below if you are setting up a new PRISM user, making changes to or deleting a current user's account. If you have any questions, please call FIS Customer Assessment at 412-624-6580. Once the forms are completed and signed where necessary, fax forms to 412-904-5502 or send to FIS Customer Assessment, 1917 Cathedral of Learning.

PRISM Central User Access forms are to only be completed by the following departments: Financial Information Administration, Budget and Financial Reporting, General Accounting, Research and Cost Accounting, Payment Processing, Purchasing Services, Financial Records Services, Payroll, Internal Audit, Office of Finance, Student Accounting and Billing, Student Payment Center, University Collections and Office of Research.

PRISM Archive Data Access forms are to be completed for all new and existing departmental users in order to access General Ledger Archived data only. This access is inquiry only. Please refer to the PRISM General Ledger Archive.
